Any changes will be communicated via Facebook page or the closed WhatsApp group.

By sending this form, I declare, I have been fully briefed on this risk assessment and other control measures in place to reduce the risk of injury to the lowest possible level. I fully understand my duties, to follow the control measures in this risk assessment and the method statement.

Insurance *

I am aware, that the the NHA insurance does not cover any impact of a Covid-19 infection. I hereby waive any claims against Newquay Handball Club and its officials. However, that does not include claims against individuals, causing infections due to careless or intentional behaviour.

Covid-19 Symptoms *

I hereby declare, that I do not show any Covid-19 Symptoms by the time of sending this form. I will report any change immediately and I will not attend any session if I do show symptoms. I am aware, that ignoring this could result in personal claims against me.
